0186(1) this licensor observed a completed individual care plan for the child. 0186(3) this licensor spoke to multiple staff and the center followed the individual care plan during the incident. 0186 (4) is valid because staff stated they did not review the child's individual care plan with the allergy list prior to serving the food the day of the incident.
